Objective:
To evaluate temporal trends in the prevalence of gram-negative bacteria (GNB) with difficult-to-treat resistance (DTR) in the southeastern United States. Secondary objective was to examine the use of novel β-lactams for GNB with DTR by both antimicrobial use (AU) and a novel metric of adjusted AU by microbiological burden (am-AU).
Design:Retrospective, multicenter, cohort.
Setting:Ten hospitals in the southeastern United States.
Methods:GNB with DTR including Enterobacterales, Pseudomonas aeruginosa, and Acinetobacter spp. from 2015 to 2020 were tracked at each institution. Cumulative AU of novel β-lactams including ceftolozane/tazobactam, ceftazidime/avibactam, meropenem/vaborbactam, imipenem/cilastatin/relebactam, and cefiderocol in days of therapy (DOT) per 1,000 patient-days was calculated. Linear regression was utilized to examine temporal trends in the prevalence of GNB with DTR and cumulative AU of novel β-lactams.
Results:The overall prevalence of GNB with DTR was 0.85% (1,223/143,638) with numerical increase from 0.77% to 1.00% between 2015 and 2020 (P = .06). There was a statistically significant increase in DTR Enterobacterales (0.11% to 0.28%, P = .023) and DTR Acinetobacter spp. (4.2% to 18.8%, P = .002). Cumulative AU of novel β-lactams was 1.91 ± 1.95 DOT per 1,000 patient-days. When comparing cumulative mean AU and am-AU, there was an increase from 1.91 to 2.36 DOT/1,000 patient-days, with more than half of the hospitals shifting in ranking after adjustment for microbiological burden.
Conclusions:The overall prevalence of GNB with DTR and the use of novel β-lactams remain low. However, the uptrend in the use of novel β-lactams after adjusting for microbiological burden suggests a higher utilization relative to the prevalence of GNB with DTR.
COVID-19 passenger screening to reduce travel risk and translocation of disease

Aviation passenger screening has been used worldwide to mitigate the translocation risk of SARS-CoV-2. We present a model that evaluates factors in screening strategies used in air travel and assess their relative sensitivity and importance in identifying infectious passengers. We use adapted Monte Carlo simulations to produce hypothetical disease timelines for the Omicron variant of SARS-CoV-2 for travelling passengers. Screening strategy factors assessed include having one or two RT-PCR and/or antigen tests prior to departure and/or post-arrival, and quarantine length and compliance upon arrival. One or more post-arrival tests and high quarantine compliance were the most important factors in reducing pathogen translocation. Screening that combines quarantine and post-arrival testing can shorten the length of quarantine for travelers, and variability and mean testing sensitivity in post-arrival RT-PCR and antigen tests decrease and increase with the greater time between the first and second post-arrival test, respectively. This study provides insight into the role various screening strategy factors have in preventing the translocation of infectious diseases and a flexible framework adaptable to other existing or emerging diseases. Such findings may help in public health policy and decision-making in present and future evidence-based practices for passenger screening and pandemic preparedness.
P114: Experiences of nursing home residents with dementia and chronic pain using an interactive social robot: A qualitative study of multiple stakeholders

Objective:
To explore the benefits and barriers of using an interactive robotic seal (PARO, Figure 1) based on the experiences of nursing home residents living with dementia and chronic pain, their family members, and formal caregivers.
Methods:Semi-structured interviews were conducted alongside a feasibility randomized controlled trial at one nursing home in Brisbane, Australia between July 2021 and January 2022 (Trial registration: ACTRN 12621000837820). Residents with dementia and chronic pain interacted with PARO individually for 15 min once or twice daily, five days per week for three consecutive weeks. After which, individual interviews were conducted with residents who were capable of communicating (n=13), family members (n=3), registered nurses (n=4), care assistants (n=11), a physical therapist (n=1), a diversional therapist (n=1) and the facility manager (n=1) who experienced or observed the residents’ interactions with PARO. The interviews were audio-recorded, transcribed, and analyzed using thematic analysis.
Results:Almost all participants reported that interacting with PARO benefited residents with dementia and their caregivers. These benefits included (1) reducing pain by providing distraction and stimulation; (2) reducing behavioral and psychological symptoms of dementia; (3) promoting positive emotions by recalling memories; and (4) reducing anxiety and care burden for family and formal caregivers. Neutral attitudes toward PARO were reported by three residents with mild cognitive impairment as they reported it did not make any difference. Barriers to using PARO included limited staff training and the implementation of person‐ centered care due to limited resources.
Conclusion:Overall, multiple stakeholders were positive about using PARO to reduce pain and behavioral symptoms of nursing home residents living with dementia and chronic pain. PARO may also reduce the care burden of family and formal caregivers. PARO might be incorporated into daily practice to support nursing home residents living with dementia. Improving staff training and understanding individual preferences of residents may enhance the implementation of PARO in this population.

Background
Randomised controlled trials (RCTs) of psilocybin have reported large antidepressant effects in adults with major depressive disorder and treatment-resistant depression (TRD). Given psilocybin's psychedelic effects, all published studies have included psychological support. These effects depend on serotonin 2A (5-HT2A) receptor activation, which can be blocked by 5-HT2A receptor antagonists like ketanserin or risperidone. In an animal model of depression, ketanserin followed by psilocybin had similar symptomatic effects as psilocybin alone.
AimsTo conduct a proof-of-concept RCT to (a) establish feasibility and tolerability of combining psilocybin and risperidone in adults with TRD, (b) show that this combination blocks the psychedelic effects of psilocybin and (c) provide pilot data on the antidepressant effect of this combination (compared with psilocybin alone).
MethodIn a 4-week, three-arm, ‘double dummy’ trial, 60 adults with TRD will be randomised to psilocybin 25 mg plus risperidone 1 mg, psilocybin 25 mg plus placebo, or placebo plus risperidone 1 mg. All participants will receive 12 h of manualised psychotherapy. Measures of feasibility will include recruitment and retention rates; tolerability and safety will be assessed by rates of drop-out attributed to adverse events and rates of serious adverse events. The 5-Dimensional Altered States of Consciousness Rating Scale will be a secondary outcome measure.
ResultsThis trial will advance the understanding of psilocybin's mechanism of antidepressant action.
ConclusionsThis line of research could increase acceptability and access to psilocybin as a novel treatment for TRD without the need for a psychedelic experience and continuous monitoring.

The Variables and Slow Transients Survey (VAST) on the Australian Square Kilometre Array Pathfinder (ASKAP) is designed to detect highly variable and transient radio sources on timescales from 5 s to $\sim\!5$ yr. In this paper, we present the survey description, observation strategy and initial results from the VAST Phase I Pilot Survey. This pilot survey consists of $\sim\!162$ h of observations conducted at a central frequency of 888 MHz between 2019 August and 2020 August, with a typical rms sensitivity of $0.24\ \mathrm{mJy\ beam}^{-1}$ and angular resolution of $12-20$ arcseconds. There are 113 fields, each of which was observed for 12 min integration time, with between 5 and 13 repeats, with cadences between 1 day and 8 months. The total area of the pilot survey footprint is 5 131 square degrees, covering six distinct regions of the sky. An initial search of two of these regions, totalling 1 646 square degrees, revealed 28 highly variable and/or transient sources. Seven of these are known pulsars, including the millisecond pulsar J2039–5617. Another seven are stars, four of which have no previously reported radio detection (SCR J0533–4257, LEHPM 2-783, UCAC3 89–412162 and 2MASS J22414436–6119311). Of the remaining 14 sources, two are active galactic nuclei, six are associated with galaxies and the other six have no multi-wavelength counterparts and are yet to be identified.

Summary
Introduction
This chapter presents the outlines of a constructivist understanding of world politics. We begin with a discussion of state identity, a fundamental concept of constructivism applied to international affairs, and explore the ways in which identity defines and bounds state actions. To illustrate this concept, we address a number of issues central to the study of world politics: change, governance and security. Overall, our goal is to present a thickly textured, layered understanding of the international realm based on a notion taken for granted in much of IR theory: meaning.
Constructivism is the newest but perhaps the most dynamic of the main theories of international relations. The important works heralding the constructivist approach to the study of global politics – articles by Alexander Wendt (1987, 1992) and books by Nicholas Onuf (1989) and Friedrich Kratochwil (1989) – are only about two and a half decades old, even though the intellectual traditions on which they draw have long histories in other academic fields. Unlike liberalism and realism (see Chapters 2 and 3), which have taken their bearings from developments in economic and political theory, constructivism – like critical theory (see Chapter 4) – is rooted in insights from social theory (e.g. Berger and Luckmann 1967; Giddens 1984) and the philosophy of knowledge (Golinski 2005; Hacking 1999; Searle 1995). Perhaps in consequence, constructivism does not predict outcomes, or offer definitive advice on how a state should act in the international arena. Instead, constructivism is best understood as a set of wagers about the way that social life is put together – wagers that centrally revolve around the fundamental importance of meaning to social action: ‘people act toward objects, including each other, on the basis of the meanings those objects have for them’ (Wendt 1999: 140). Constructivist IR theory is an application of that basic analytical commitment to the study of global politics, focusing in the first instance on state identity.
What does constructivism do? Identity and international institutions
So what exactly are the basic tenets of constructivist IR? This is a very hard question to answer because, as a relatively new theory, there has not been as much time for people to work out in detail what the most central propositions of the constructivist way of doing things are.

Data from the Seasonal Ice Zone Observing Network (SIZONet) acquired near Barrow, Alaska, during the 2009/10 ice season allow novel comparisons between measurements of ice thickness and velocity. An airborne electromagnetic survey that passed over a moored Ice Profiling Sonar (IPS) provided coincident independent measurements of total ice and snow thickness and ice draft at a scale of 10 km. Once differences in sampling footprint size are accounted for, we reconcile the respective probability distributions and estimate the thickness of level sea ice at 1.48 ± 0.1 m, with a snow depth of 0.12 ± 0.07 m. We also complete what we believe is the first independent validation of radar-derived ice velocities by comparing measurements from a coastal radar with those from an under-ice acoustic Doppler current profiler (ADCP). After applying a median filter to reduce high-frequency scatter in the radar-derived data, we find good agreement with the ADCP bottom-tracked ice velocities. With increasing regulatory and operational needs for sea-ice data, including the number and thickness of pressure ridges, coordinated observing networks such as SIZONet can provide the means of reducing uncertainties inherent in individual datasets.

Here, we report reproducible and accurate measurement of crystallographic parameters using scanning transmission electron microscopy. This is made possible by removing drift and residual scan distortion. We demonstrate real-space lattice parameter measurements with <0.1% error for complex-layered chalcogenides Bi2Te3, Bi2Se3, and a Bi2Te2.7Se0.3 nanostructured alloy. Pairing the technique with atomic resolution spectroscopy, we connect local structure with chemistry and bonding. Combining these results with density functional theory, we show that the incorporation of Se into Bi2Te3 causes charge redistribution that anomalously increases the van der Waals gap between building blocks of the layered structure. The results show that atomic resolution imaging with electrons can accurately and robustly quantify crystallography at the nanoscale.

Purpose: There is limited research on the costs of social and emotional learning (SEL) interventions [Crowley, Jones, Greenberg, Feinberg & Spoth (2012). Resource Consumption of a Diffusion Model for Prevention Programs: The PROSPER Delivery System. Journal of Adolescent Health, 50 (3), 256–263]. This paper describes a comprehensive methodology for determining the costs of a successful universal, school-based SEL intervention that was implemented in nine public schools over 3 years.
Methods: Resource costs were identified using the Cost–Procedure–Process–Outcome Analysis Model [Yates (1996). Analyzing Costs, Procedures, Processes, and Outcomes in Human Services. Thousand Oaks, CA, US: Sage Publications, Inc.; Yates (1999). Measuring and Improving Cost, Cost-Effectiveness, and Cost-Benefit for Substance Abuse Treatment Programs. No. NIH 99-4518, 135] and the ingredients model [Levin (Ed.) (1983). Cost-Effectiveness A Primer (Vol. 4). Beverly Hills, CA: Sage; Levin & McEwan (2001). Cost-Effectiveness Analysis: Methods and Applications. (2nd ed.). Thousand Oaks, CA: Sage Publications]. This involved careful identification of resource use, finding the cost per unit for each resource by intervention activity, and ultimately calculating the total resource cost (resource use $\times$ cost per unit).
Results: Our analysis estimated the overall cost of this 3-year SEL and literacy intervention to be $1,831,296 for nine schools. This averages to $67,825 yearly per school and $130 yearly for each student. The analysis estimated the first year of the intervention to be the costliest ($683,106) and then decreasing in Year 2 ($581,764) and Year 3 ($566,426).
Conclusion: This research emphasizes the need to study the costs of SEL interventions. By providing a detailed and standardized methodology, this cost analysis can provide added support for implementing an effective social and emotional learning intervention in a school setting. Furthermore, it provides groundwork for more advanced cost analyses, such as a cost–effectiveness analysis or a benefit-cost analysis (BCA).

In this article we develop a test to determine whether a sextic polynomial that is irreducible over $\mathbb{Q}$ has Galois group isomorphic to the alternating group ${{A}_{4}}$. This test does not involve the computation of resolvents, and we use this test to construct several infinite families of such polynomials.

Illegal exploitation of resources is a cause of environmental degradation worldwide. The effectiveness of conservation initiatives such as marine protected areas relies on users' compliance with regulations. Although compliance can be motivated by social norms (e.g. peer pressure and legitimacy), some enforcement is commonly necessary. Enforcement is expensive, particularly in areas far from land, but costs can be reduced by optimizing enforcement. We present a case study of how enforcement could be optimized at Cocos Island National Park, Costa Rica, an offshore protected area and World Heritage Site. By analysing patrol records we determined the spatial and temporal distribution of illegal fishing and its relationship to patrol effort. Illegal fishing was concentrated on a seamount within the Park and peaked during the third year-quarter, probably as a result of oceanographic conditions. The lunar cycle in conjunction with the time of year significantly influenced the occurrence of incursions. The predictability of illegal fishing in space and time facilitates the optimization of patrol effort. Repeat offenders are common in the Park and we suggest that unenforced regulations and weak governance are partly to blame. We provide recommendations for efficient distribution of patrol effort in space and time, establishing adequate governance and policy, and designing marine protected areas to improve compliance. Our methods and recommendations are applicable to other protected areas and managed natural resources.

ObjectiveDescribe the epidemiology of carbapenem-resistant Enterobacteriaceae (CRE) and examine the effect of lower carbapenem breakpoints on CRE detection.
DesignRetrospective cohort.
SettingInpatient care at community hospitals.
PatientsAll patients with CRE-positive cultures were included.
MethodsCRE isolated from 25 community hospitals were prospectively entered into a centralized database from January 2008 through December 2012. Microbiology laboratory practices were assessed using questionnaires.
ResultsA total of 305 CRE isolates were detected at 16 hospitals (64%). Patients with CRE had symptomatic infection in 180 cases (59%) and asymptomatic colonization in the remainder (125 cases; 41%). Klebsiella pneumoniae (277 isolates; 91%) was the most prevalent species. The majority of cases were healthcare associated (288 cases; 94%). The rate of CRE detection increased more than fivefold from 2008 (0.26 cases per 100,000 patient-days) to 2012 (1.4 cases per 100,000 patient-days; incidence rate ratio (IRR), 5.3 [95% confidence interval (CI), 1.22–22.7]; P = .01). Only 5 hospitals (20%) had adopted the 2010 Clinical and Laboratory Standards Institute (CLSI) carbapenem breakpoints. The 5 hospitals that adopted the lower carbapenem breakpoints were more likely to detect CRE after implementation of breakpoints than before (4.1 vs 0.5 cases per 100,000 patient-days; P < .001; IRR, 8.1 [95% CI, 2.7–24.6]). Hospitals that implemented the lower carbapenem breakpoints were more likely to detect CRE than were hospitals that did not (3.3 vs 1.1 cases per 100,000 patient-days; P = .01).
ConclusionsThe rate of CRE detection increased fivefold in community hospitals in the southeastern United States from 2008 to 2012. Despite this, our estimates are likely underestimates of the true rate of CRE detection, given the low adoption of the carbapenem breakpoints recommended in the 2010 CLSI guidelines.

Summary
Introduction
This chapter presents a constructivist understanding of world politics. We begin with a discussion of state identity, a fundamental concept of constructivism, and explore the way that identity defines and bounds state actions. To illustrate this concept, we address a number of issues central to the study of world politics: change, governance, and security. Overall, our goal is to present a thickly textured, layered understanding of the international realm based on a notion taken for granted in much of IR theory, meaning.
Constructivism is the newest but perhaps the most dynamic of the main theories of international relations. The seminal works inaugurating the constructivist approach to the study of global politics – articles by Alexander Wendt (1987; 1992) and books by Nicholas Onuf (1989) and Friedrich Kratochwil (1989) – are only about two and a half decades old, even though the intellectual traditions on which they draw have long histories in other academic fields. Unlike liberalism and realism (see Chapters 2 and 3), which have taken their bearings from developments in economic and political theory, constructivism, like Critical Theory (see Chapter 4), is rooted in insights from social theory (e.g. P. Berger and Luckmann 1967; Giddens 1984) and the philosophy of knowledge (Searle 1995; Hacking 1999; Golinski 2005). Perhaps in consequence, constructivism does not predict events, or offer definitive advice on how a state should act in the international arena. Instead, constructivism is best understood as a set of wagers about the way that social life is put together, wagers that centrally revolve around the fundamental importance of meaning to social action: ‘people act toward objects, including each other, on the basis of the meanings those objects have for them’ (Wendt 1999: 140). Constructivist IR theory is an application of that basic analytical commitment to the study of global politics.

Summary
The psychiatric report for legal purposes is the closest professional link between the two disciplines of psychiatry and law. Clarifying the consultation question is a vital component of effective forensic psychiatric report writing. In some reports, a single section briefly describing the subject's personal history may suffice. In others, a more comprehensive and well-delineated breakdown of the personal history into separate sections will provide clarity and detail necessary to support more complex conclusions. The psychiatrist recognizes many components of a clinical psychiatric evaluation. The forensic psychiatric report allows the expert witness to perform a teaching function using their psychiatric education, training, and experience to put psychiatric information and conclusions in a legal context. The forensic psychiatrist uses the report to explain to the legal system how the specific manifestations of the mental illness affected the subject's thinking, judgment, and behavior.

School-based strategies to prevent violence, trauma, and psychopathology: The challenges of going to scale

Children's trauma-related mental health problems are widespread, largely untreated and constitute significant barriers to academic achievement and attainment. Translational research has begun to identify school-based interventions to prevent violence, trauma and psychopathology. We describe in detail the findings to date on research evaluating one such intervention, the Reading, Writing, Respect, and Resolution (4Rs) Program. The 4Rs Program has led to modest positive impacts on both classrooms and children after 1 year that appear to cascade to more impacts in other domains of children's development after 2 years. This research strives not only to translate research into practice but also translate practice into research. However, considerable challenges must be met for such research to inform prevention strategies at population scale.
